Definition
Form is an official document containing questions and spaces for answers. It is a
document (printed or electronic) with spaces in which we can write or enter data.
There are many kinds of forms;
- School/ course registration form
- Scholarship application form
- Application form for opening bank account
- application form for making identity card
- Curriculum vitae
Function
To collect someone’s personal information in order to fulfill requirements that are
asked by companies or institutions
1. To record a personal enquairy
2. To applay for something
3. To make reservation
4. To ask people for their opinion (Questionnaires)
Registration Form
Registration form is a field that has some spaces for personal identities. t aims to
collect individual or groups identities. In some situations, a registration form is need-
ed to sign people up to their services, program or events.

Job Application
A letter of application, also known as a cover letter, is a document sent with your
resume to provide additional information about your skills and experience. The let-
ter of application is intended to provide detailed information on why are you are a
qualified candidate for the job you are applying for.

Order/ Reservation Form
An order form is a document filled in by customers when asking for goods to order
something easily and simplify or remember something that has been ordered.
